More FOIA request to the High School for imaginary form

Since I got nowhere with my 1st batch of record requests about a form the school says they have but I just can't seem to get a copy of it. I request the process and procedure of how all school permission slips / forms / etc that require a parents signature are made into policy. Lets see if I have any luck this time.
